What you agree to when creating a Sergistry account.
Sergistry is a hospital registration and surgical registry platform. By creating an account you agree to provide accurate information about yourself and the organization you represent, and to submit only documents you are authorized to share.
Hospital applications are reviewed by platform administrators. Submitting fraudulent documents or registering an organization without authority will lead to rejection and may be reported.
By registering, you confirm that you are authorized to register a hospital on behalf of your organization.
Your acceptance of these terms is recorded with a timestamp, IP address, and browser signature.
